41 Appendix Survey Form The attached form shows all questions asked of any survey participant. However, the survey contained skip logic that hid some questions from respondents depending on responses to prior questions, so all respondents were not asked every question.

42 Overview The Airport Cooperative Research Program (ACRP) is conducting a study on the challenges and opportunities that Transportation Network Companies (TNCs) such as UberX and Lyft present for airport operators. The rise of TNCs has brought about a quickly evolving transportation business environment. The objective of this research is to compile experiences and effective practices by airports in accommodating and regulating TNCs, and the effects of their decisions on airport operations and finance. If TNCs are not yet operating at your airport, we encourage you to still complete the survey, as this is also important information. The survey logic will automatically skip irrelevant questions based on previous responses. We expect it will take less than 3 minutes to complete the survey if you do NOT have TNCs operating at your airport and up to 15 minutes if you DO have an agreement in place with TNCs. All survey data will be aggregated to maintain anonymity unless you elect to have your airport be considered a case study. If you are selected as a case study, we will follow up with you to confirm any information to be included prior to publication.
