Appendix A

Statement of Task

The National Academies of Sciences, Engineering, and Medicine shall establish an ad hoc planning committee to plan and host a 1- to 2-day public workshop to facilitate a discussion focused on the use of biomarkers to establish the presence and severity of disability.

The meeting will feature invited presentations and discussions on topics such as:

  1. A general overview of biomarkers and the current and potential purposes for their use, including presentations which explain:
    1. The accuracy of the definitions of key terms in biomarker research, to be provided by the Social Security Administration (e.g., clinical endpoint, surrogate endpoint, telomere);
    2. The difference between the term “biomarker” (which may include traits or metrics such as height or blood pressure) from the more specific “molecular biomarker” for which the term is typically used as an abbreviation;
    3. The types of diagnostic, non-genetic biomarkers (e.g., distinct anatomical or physiological signatures); and
    4. The range of impairments for which health care professionals currently use diagnostic and prognostic biomarkers and their potential for use in the future.
  2. How health care professionals now use non-genetic biomarkers as diagnostic/prognostic tools and severity indicators in the following physical and mental impairments (whether as a sole indicator

  1. of the presence of impairment or as confirmation of a clinically significant sign associated with the impairment), and summarize the supporting research:

    1. Fibromyalgia;
    2. Arthritis;
    3. Post-traumatic stress disorder;
    4. Major depression;
    5. Schizophrenia; and
    6. Chronic pain.
  2. Whether it is typical for health care professionals to:
    1. Associate any biomarker with a particular level of functional status; and
    2. Use any biomarker as a sole indicator of impairment severity.
  3. The legal and ethical implications associated with biomarker use in clinical decision making.

A proceedings of the presentations and discussions at the workshop will be prepared by a designated rapporteur in accordance with institutional guidelines.

As part of the overall disability determination process, the Social Security Administration uses a step-by-step approach to understand how severe an individual's condition is and whether it meets program criteria for disability. The use of various types of biomarkers has been suggested as a way to strengthen the amount and quality of objective evidence available to the review process.

At the request of the Social Security Administration, the National Academies of Sciences, Engineering, and Medicine's Board on Health Care Services organized a virtual workshop on July 21, 2020, titled The State of the Science of the Use of Biomarkers to Establish the Presence and Severity of Impairments. Workshop discussions focused on the current and potential uses for biomarkers; explored legal and ethical implications associated with biomarker use in clinical decision making; and considered the possible uses of nongenetic biomarkers as tools for the diagnosis or prognosis of the severity of specific physical and mental impairments. This publication summarizes the presentations and discussion of the workshop.
