101 APPENDIX A: Survey Questionnaire NCHRP 20-07 - Work Zone Intrusion Technology Survey Alternative Technologies for Mitigating the Risk of Injuries and Deaths in the Work Zone You are being invited to participate in a survey about WZITs used to prevent accidents in work zones. WZITs are technologies used in a work zone to prevent vehicle intrusion into the work zone, mitigate the impacts of an intrusion, and/or warn workers and drivers of an intrusion. The survey is being conducted by Oregon State University as part of a study funded by the NCHRP. What will happen during this study and how long will it take? You will be asked to express your opinion and share your experience related to the extent of use and effectiveness of WZIT in roadway construction. You will also be asked your opinion about strategies for adopting WZIT and inhibitors associated with the adoption. Lastly, you will be asked whether you are willing to provide additional information about WZIT to the researchers through a more in-depth, follow-up interview. You may print or take a screenshot of the consent page for your records. It is expected that the survey will take approximately 10-15 minutes to complete. The information you provide is for research purposes only. Some of the questions are personal. You can choose not to answer any of the questions if you wish How will my information be used? Your responses to the survey questions will be summarized in a final research report and one or more academic papers. All publications of the study results will not include any information about your identity or affiliation. A summary of the results will be created for the benefit of the research team. However, the summary will not include personal information of the participants (e.g., name, title, company, etc.). All information provided will be kept strictly confidential and viewed only by the researchers and the funding agency (National Academy of Sciences). Your information and responses that are collected as part of the research, even if identifiers are removed, will not be used or distributed for future research studies. Survey responses will be provided to the National Academy of Sciences as part of the study requirements, however the data provided will not include any identifying information such as personal and company/organization names or other identifying information.
