The Oklahoma Clean Water Inspection Form contains the Oklahoma D O T logo and title at the top, with fields to enter date, job piece, project number, contract I D, location description, R E and or O D O T contact, and type of inspection. The form provides checklists with yes, no, and not applicable options for multiple checks, which are grouped into categories: erosion minimization, sediment discharge minimization, best management practice maintenance, solid and hazardous waste, documentation and S W P P P, stabilization, and additional information. Each checklist has space for comments. At the bottom, a certification statement affirms the accuracy of the inspection, followed by spaces for the inspector name and signature, reviewer name and signature, and reviewer comments. A note at the bottom reads: The items in this inspection form align with the commitments of the O K R 10. The most recent iteration of this permit, along with supporting documents, can be found through the Q R code at the top.

State department of transportation (DOT) construction activities must comply with federal and state National Pollutant Discharge Elimination System construction general permit requirements. The regulations require state DOTs to implement policies, procedures, and practices to minimize offsite discharges of sediment-laden stormwater to protect downstream receiving water bodies during construction.
