The West Virginia D O T Division of Highways Environmental Construction Inspection Form includes fields at the top for project information such as inspection date, inspector name, project number, and weather conditions. Below, a table lists practices to be assessed for compliance with columns labeled yes, no, not applicable, and a column for notes or descriptions of notable issues with their risk ratings. The next section has yes or no questions about unpermitted discharge and offsite sediment discharge from the site, followed by a section for notes from the current inspection with a large blank area for comments or descriptions. The bottom of the form contains signature lines for the inspector and reviewer, plus a table listing the action risk rating scale that includes the rating level, risk level, corrective action timeframe, and examples.

State department of transportation (DOT) construction activities must comply with federal and state National Pollutant Discharge Elimination System construction general permit requirements. The regulations require state DOTs to implement policies, procedures, and practices to minimize offsite discharges of sediment-laden stormwater to protect downstream receiving water bodies during construction.
