The Colorado D O T stormwater field inspection report for active construction has fields for project name, project contractor, stormwater manager, project engineer, attendees, project number, sub account, permit certification, project region, inspection date, and weather at the time of inspection. The next section lists reasons for inspection or exclusion with definitions for routine inspection, runoff event, winter conditions exclusion, and an area for other reasons. A checklist section for stormwater management has questions about the stormwater management plan location, approval, inspection reports, corrective actions, spill response plan, and pollutant list, with yes, no, and not applicable columns and space for reasons. The following section asks for a description of current construction activities and an estimate of disturbed area with fields for temporary stabilization, interim stabilization, permanent stabilization, other, and total disturbed acres. There is a question about the status of the control measure matrix. Next, a large table is provided for recording site assessment and corrective actions, including location, control measure, condition, comments, and date completed with initials. The next section assesses whether sediment or other pollutants have left the site or reached state waters with yes or no checkboxes and space for notes. General notes and a large blank space for comments are included. Certification sections for inspection and compliance have fields for printed names, required signatures, and dates for the stormwater manager, erosion control inspector, superintendent, and project engineer. The bottom part contains a block of instructions that defines state waters, responsibilities of stormwater managers and engineers, documentation and retention requirements, and steps for reviewing the stormwater management plan, completing the inspection, and documenting corrective actions.

State department of transportation (DOT) construction activities must comply with federal and state National Pollutant Discharge Elimination System construction general permit requirements. The regulations require state DOTs to implement policies, procedures, and practices to minimize offsite discharges of sediment-laden stormwater to protect downstream receiving water bodies during construction.
