The Iowa D O T storm water oversight quality assurance inspection form is depicted, containing sections for inspection date and time, D N R authorization, project and county details, prime contractor name, inspection made by, and date of previous Q A inspection. Spaces are provided for listing contractor staff and D O T staff, including erosion control technicians with fields for certification and expiration dates, as well as E S C basics training status. There are documentation review checkboxes for the erosion control implementation plan, E C I P updates, and subcontractor co-permittee statements, followed by areas for comments. Inspection reports review includes checkboxes for missing reports and lacking information, along with a space for comments. Next is a section labeled Deficiencies found that lists ten items: silt fencing, ditch checks, sediment control, rock, silt basin, seeding and mulch, housekeeping, discharge locations, and two spaces for other items. Each deficiency item has checkboxes for whether corrective action is required, and a space for comments, recommended improvements, and location. Multiple horizontal lines allow for entering further item numbers, comments, and locations. At the bottom is a certification statement for document accuracy, with lines for the inspector’s signature and date. An attached instructions page provides detailed guidance on documenting current construction activities, estimating disturbed area, updating and reviewing the phased control measure implementation matrix, inspecting and identifying site deficiencies, assigning control measures, documenting corrective actions, and removing unnecessary controls. There is emphasis on identifying inadequate or additional measures and ensuring proper reporting for sediment or pollutant discharges. The final part addresses general notes for the inspector, a section for inspection certification to verify attendance, and compliance certification required after all corrections are made, referencing regulatory requirements.

State department of transportation (DOT) construction activities must comply with federal and state National Pollutant Discharge Elimination System construction general permit requirements. The regulations require state DOTs to implement policies, procedures, and practices to minimize offsite discharges of sediment-laden stormwater to protect downstream receiving water bodies during construction.

NCHRP Synthesis 648: Construction Stormwater Program Management, Tracking, Reporting, and Compliance, from TRB's National Cooperative Highway Research Program, documents state DOT practices, tools, and approaches for managing compliance with state and federal construction stormwater permit requirements.
