The Texas D O T construction stormwater pollution prevention plan field inspection and maintenance report forms are depicted. The top section shows project information fields, including inspection date, authorization numbers, rainfall data, and county. Below, checkboxes for inspection cycles and an extensive checklist of Best Management Practice or B M P areas, such as disturbed areas, discharge locations, erosion and sediment control B M Ps, truck wash, and material storage areas, are listed for inspection. There are sections for documenting corrective actions, maintenance, upgrading, or additional controls, with tables to fill out the B M P or area, station or location, issue, cause, corrective action, priority, and verification initials. An additional section for temporary and permanent stabilization follows, with fields for area number, sheet number, phase, locations, activity dates, cover achievement, measures, and notes. Another table is provided for observations, with space to enter the B M P number, station or location, side of the centerline, comments, and reminders. State department of transportation (DOT) construction activities must comply with federal and state National Pollutant Discharge Elimination System construction general permit requirements. The regulations require state DOTs to implement policies, procedures, and practices to minimize offsite discharges of sediment-laden stormwater to protect downstream receiving water bodies during construction.
