Appendix A Statement of Task An ad hoc committee will plan, organize, and host a 1-day public workshop to examine issues pertaining to integrating the voice of patient and family caregivers into the delivery of care for people of all ages with serious illness. The workshop will feature invited presentations and panel discussions on topics that may include • Examination of the current challenges and needs related to integrat ing the patient and caregiver voice into serious illness care • Shared decision making in the context of serious illness care • Ways to enhance patient knowledge and understanding of treatment options • Ways to facilitate and support engagement of the patient and family caregivers in the decisions regarding serious illness care • Evidence gaps and challenges that may impede the integration of patient and family preferences into serious illness care, and oppor tunities to address them • Disparities in care related to social determinants of health, includ ing factors such as age, race, ethnicity, socioeconomic status, and geography • Unique needs of pediatric patients and families 73

... 74 PATIENT AND CAREGIVER VOICE IN SERIOUS ILLNESS CARE The planning committee will develop the agenda for the workshop, select speakers and discussants, and moderate the discussions. Proceedings of the presentations and discussions at the workshop will be prepared by a designated rapporteur in accordance with institutional guidelines.
